Notice of Funding Availability: RFA #20510 – Hepatitis C Navigation and Care Coordination in High-Risk Settings – Reissue

The New York State Department of Health (NYSDOH) AIDS Institute (AI), Division of HIV and Hepatitis Health Care/Bureau of Hepatitis Health Care and Epidemiology announces the availability of New York State funds to increase the number of people living with the hepatitis C virus (HCV) who are linked to hepatitis C care, initiate and complete treatment and are cured of hepatitis C, and reduce disparities associated with treatment access. The intent of the Request for Applications (RFA) is to fund $1,700,000 annually through October 31, 2028.

This RFA contains two Components:

Component A: Hepatitis C Navigation Services in High-Risk Settings
Component A funding will support patient navigation services for clients served in high-risk settings.

Component B: Nurse Care Coordination in Opioid Treatment Programs
Component B funding will support nurse care coordination services at opioid treatment programs providing hepatitis C treatment onsite or via telehealth through a memorandum of understanding (MOU) with an experienced hepatitis C provider.

In order to apply for this Request for Applications, eligible applicants must be prequalified in the Statewide Financial System and must submit an application via the Statewide Financial System.

Applicants may submit separate applications for each Component. However, no more than one (1) application per component will be accepted in response to this Request for Applications (RFA).

Late applications will not be accepted.
Applications will not be accepted via fax, e-mail, hard copy or hand delivery.

Written questions will be accepted until the date posted on the Cover Page of the Request for Application.
